We know that visiting a doctor’s office is often a frustrating experience. You have to get to the office, wait in the waiting room, and then often wait again to see a doctor who doesn’t spend time listening and getting to know you. At Heyday Health, we come to you.
Our telehealth appointments are over video, like Zoom or Facetime, or on the phone. We also see all patients in their homes, so we can spend time with you in person.
Every patient at Heyday Health has a three-person care team – a doctor, a nurse practitioner, and a health ally. You will always talk to and see your own care team, so that you always feel comfortable with the people who are responsible for your health.
Our Health Allies help you with things like scheduling appointments, getting prescriptions, coordinating physical therapy or other therapeutic needs, and supporting your health as a Heyday patient. When you’re a patient at Heyday Health, your Health Ally is the first person you talk to about your care and to make sure you get everything you need to stay healthy.
We know that loved ones and caregivers often play an important role in supporting patients’ health and care plans. With Heyday Health, you can choose to have your loved ones attend in-home appointments. They can also participate in video and phone appointments, whether they live down the street or across the country. You can choose to have your loved ones receive updates on your health, medications, and test results and they can communicate with your care team to understand and help support your health.
